Participant Waiver, Release of Liability, and Assumption of Risk

This Agreement is intended to protect Take5athletics, its owners, coaches, trainers, contractors, volunteers, and property (the “Released Parties”) from liability related to participation in training sessions, classes, or events conducted at the Take5athletics home-based training facility or any other location used by Take5athletics.

Acknowledgment of Risks

I acknowledge that athletic training, conditioning, and related activities including but not limited to strength training, speed and agility drills, plyometrics, use of resistance and cardio equipment, mobility exercises, and sport-specific movements carry inherent risks of injury.

Medical Clearance

I certify that I am physically fit, have no medical condition that would prevent safe participation, and have either obtained medical clearance from a physician or chosen to participate at my own risk.

Assumption of Risk

I knowingly and voluntarily assume all known and unknown risks associated with participation, including those arising from the negligence of the Released Parties.

Waiver and Release of Liability

In consideration for being allowed to participate, I waive, release, discharge, and covenant not to sue Take5athletics or the Released Parties for any and all claims arising out of or related to my participation.
